Improving
someones quality of life can be accomplished through functional
approaches to rehabilitation designed specifically to address real
world issues - living, working, and accessing the community. When
appropriate community based rehabilitation is available, the likelihood
of success in returning to pre-accident level of activity increases.
Program
evaluation includes objective data, standardized score (established
with regulated health care professional) and a supervision plan.
Community
based programs are designed to focus on the individuals needs. Emphasis
is made on enhancing skills and activities to help the person achieve
optimal levels of independence when the opportunity to learn in
their own environment, allowing learning to be reality based, is
presented.
Community
based rehabilitation involves establishing goals with the person
who has been injured and thus involving the program in the persons
life rather than involving the persons life into the program.
Community
based support personnel work to facilitate empowerment in all aspects
of daily life. This is accomplished through a client focused, goal
oriented approach to care, involving physical, emotional, intellectual,
social and vocational needs.
