Referral Process

1. Contact Laura Gridel 905-451-1772 (x241) with referral enquiries.
2. Lawlor attempts to match the rehabilitation therapist's skills, expertise (or experience), personality, and location with the prospective client.
3. If a match and availability is established, the referral will proceed.
4. If a Treatment Plan is required, Lawlor will provide information including Section 11 (identifying the Health Providers), and Section 12 (Proposed Goods and Services).
5. Relevant Documentation should be faxed to our head office (905-451-5960).
6. The referral source will be contacted if further details are required.
7. When services are approved, the rehabilitation therapist will contact the referral source, and/or the supervising regulated health care professional to initiate therapy support services intervention.
8. An acknowledgement letter with the accompanying referral information will be forwarded to the referral source, insurer, and legal representative (if applicable).
9. Once intervention commences, the referral source and regulated health care professionals will receive weekly faxed progress notes to maintain communication among team members.
